Aflac Benefits available for all PORAC Members!

 

We are pleased to offer Aflac’s supplemental insurance plans to all PORAC members at group rates. All benefits from Aflac are paid directly to you, separate and independent of any other medical or disability insurance.  Coverage is guaranteed renewable for life, and fully portable. Available plans include:

 

 

Personal Accident Indemnity – Covers you and family for any accident, on or off the job!

• Provides cash benefits for loss of income and increased out-of-pocket expenses (such as co-pays and deductibles) for doctor visits and hospitalization.

• Includes specific-sum injury benefits based upon severity, cash for major tests and physical therapy, ambulance, accidental death benefit, and more.

• Includes an annual wellness benefit. 

• Monthly premiums start at $24.10 

 

 

Personal Short-Term Disability – Protect your income if you become ill or injured.

• Receive $500 to $5,000 per month (based on your income), tax free.

• Pays from the first day due to off-job accidents, and 14^th day due to sickness.

 

 

Personal Cancer Indemnity – Pays cash benefits at a time when needed most .

• Receive $5,000 for the first occurrence of cancer, plus the benefit builds by an additional $500 per year prior to first occurrence.

• Cash benefits for radiation, chemo, second-surgical opinions and much more; help cover loss of income during treatments, and the increased expenses that arise.

• Wellness incentives of $30 for paps, $100 for mammograms, and $75 for other tests.

• Monthly premiums start at $40.20

 

 

A Specified Health Event policy – Pays first occurrence, hospital & continuing care.

• Pays $5,000 upon the incident of heart attack, stroke, coronary bypass surgery, coma, and more; plus the benefit builds by an additional $500 per year prior to first occurrence.

• Pays income benefits of $300 per day in-patient, and $125 per day for out-patient care.

• Monthly premiums are based upon age at enrollment, starting at $21.71
